I offer evidence-based care with longer initial visits to get the full spectrum of your health needs, changes to your interventions as you progress, and longer follow-ups. Additional perks like secure chat, online bill pay, and a food/lifestyle tracking app are all included at no extra cost. DLC is HIPAA-compliant and allows communication with your doctor when needed—for instance, with medication adjustments such as insulin or diabetes medication adjustments. These do require your doctor’s approval. My approach is personalized, with high-touchpoint care and bite-sized learning, especially in the first three to six months. Sessions are interactive and tailored to your unique routine and culture.

I highly recommend asking your doctor for one to streamline communication and to help you better understand insurance coverage too. For clients (out of california), I do require a doctor’s referral. Please ask your doctor to fax referral to Diabetes & Lifestyle Care (877) 736-6591.
Do I need a doctor’s referral for meeting with you?

I provide bundled learning plans that work great for multiple visits. Studies have shown high touch point care to yield more sustainable results. It allows for continuation in care, a needed rhythm for lifestyle and behavior changes. I also make it cost effective by reducing the per visit price for bundled plans. Ask me at your initial visit for details or send me a message.
What if I need to see you more than once to achieve my goals?

I am currently not contracted with insurance companies. My services would be classified as an out of network provider for commercial insurances. This allows freedom to provide quality care independent of third party insurers as well as more time spent on direct patient care. Payments can be made through HSA/FSA card, credit card or Zelle.
Do you accept insurance?

I can provide you a superbill to submit to your insurance at no additional cost. Just make a mention at your initial visit for one. A superbill is provided post visit and any reimbursement comes directly from your insurance to you.

Please call your insurance company to know how your insurance reimburses you for dietitian services. Each insurance company is unique. Insurance will ask for the below information.
The main CPT codes for medical nutrition therapy are:
Initial visit - 97802
Follow up visit -97803
Diagnosis code - this would be on your doctor's referral or you may share your reason for visit.
Additionally, do ask your insurance about the number of visits covered per year.

What if I want to submit my bill for insurance reimbursement?

​75 minute Initial Visit - $210 (Payment due when scheduling the visit)
60 minute Follow Up Visit - $150 (Payment can be at the time of visit)
Learning plans are available for high touchpoint care providing more than a single follow up as well as a discounted rate per follow up visit

What are your fees?

Yes! The Academy of Nutrition and Dietetics supports telehealth for nutrition therapy and diabetes education. I use secure, HIPAA-compliant software, www.gethealthie.com. Outcomes are comparable to in-person sessions, Bonus: Virtual kitchen tours for hands-on learnings!

Do you also provide in-person services?

You are welcome to bring anyone you may feel important in your life as your support person as part of your health journey. Eating well is more than therapeutic and often engaging those at home who are part of the daily food arrangements can be beneficial. For child nutrition visits specifically, I prefer the initial visit with parents/caregiver only.
Can I bring my child/partner/pet to my appointment?
